BufferProvider: remove rest of the old implementation

note: regarding hash-standard.hpp see Ticket #1422
Mostly this hack worked -- but it is very likely not needed any more.
Today I had to re-wire a partial specialisation for hashing pointers,
that was added somewhere in the past into the STL.
